An Advance Health Directive (AHD) is a legal document that enables you to make decisions about the treatment you would want (or not want) to receive if you became sick or injured and were incapable of communicating your wishes.
The End of Life Care (EOLC) Program within the WA Department of Health was tasked with implementing the recommendations from the My Life My Choice report by the Joint Select Committee on End-of-Life Choices, and the subsequent Ministerial Expert Panel on Advance Health Directives: Final Report (August 2019). Specifically, the Advance Care Planning (ACP) project addresses the recommendations related to ACP including revision of the Advance Health Directive template.
This session with senior representatives from the ACP project intends to introduce the new AHD form to the legal profession.
